fix(eve): resolve pending input on message follow-up

A follow-up message now supersedes pending input instead of waiting behind it indefinitely.

Why

When a session waited for input, an unrelated user message could remain deferred until the original request received an explicit response. For tool approvals, this wedged the session even though the user had already continued the conversation.

Approach

Because a new message signals that the user has moved on, normalize every unanswered request before continuing:

• deny unanswered tool approvals
• dismiss unanswered ask_question requests with an ignored result
• preserve explicit inputResponses when a delivery contains both responses and a message
• treat message text as a follow-up even when it matches an option such as approve

The harness resolves the pending batch first, preserving provider-valid tool history, then processes the follow-up message. The approval regression test defines the prompt, pending request, transcript, and session inline. Unit coverage also verifies mixed batches and questions whose option IDs happen to be approve and deny.
