Shell completions still use `codex`

Shell statements printed out but use the codex command (also true for the tool names they contain.)

The documentation also needs an update.

Thanks for the report! We’ve updated the completion generator so it always emits the code command name (the scripts now define _code() and #compdef code, etc.), and added a regression test so the legacy codex prefix can’t sneak back in. This will land with the next build—after updating, rerun code completion <shell> and the scripts should reference code throughout. Let us know if you still spot anything off.
