Treatment of Infertility ART - Allow multiselection of planned services

Update the treatment_of_infertility_advaned_reproductive_technology_coverage encoding such that:

• the attribute claim.planned_service is renamed to claim.planned_services
• the value of the attribute is no longer a single service, but is instead a list of services from the same list
  • the metadata should be updated so that its changestyle is no longer "selector", but is instead "menu" or "checkboxfield"
  • the rules should be updated accordingly
