Enhance the oplog narrative builder to maintain formatting applied to the comments field and provide output pre-formatted as a code block.

Current Behavior
Ghostwriter intentionally strips the content of all HTML and excludes output.
Desired Behavior
Make the content potentially reusable by smartly pre-formatting it and maintain any formatting users applied to the comments. Add output, if available, as a codeblock, command as inline code, and user context as an italicized run.
Use Case
This pre-formats content will help tell more of the story and make it easy to reuse some of this content for the actual narrative. Having it pre-formatted will remove some clicks from the workflow.
