Perhaps we want to make it a bit more verbose about what failed?

I'm hitting this on a NixOS machine, where there may well be some expected package missing, or something.

I suspect it's trying read your pin, so trying to run ssh-askpass or whatever SSH_ASKPASS is pointed at. I don't have enough information to guess, but check if SSH_ASKPASS is defined and if it points to a valid binary and works on it's own?
